3) Provider had one child record with conditional immunizations and three child records with not complete immunizations and Provider did not get written proof the child is scheduled to be immunized or a signed and dated statement detailing when the child's immunizations will be brought up to date and stating they understand their child will be excluded from care if the immunizations are not completed within 30 calendar days of the specified due date within 20 business days of second initial inspection report.
